Bepaling Coronaire hartziekten: Acuut coronair syndroom

Achtergrond

Het acuut coronair syndroom (ACS) omvat acuut myocard infarct en instabiele angina pectoris. De oorzaak van een ACS is meestal een acute afname of een blokkade van de coronaire doorbloeding.1 De verwachting is dat alle patiënten met een acuut coronair syndroom bij de medisch specialist bekend zullen zijn in het jaar waarin het acuut coronair syndroom heeft plaatsgevonden. Patiënten die in een eerder jaar een acuut coronair syndroom hebben gehad kunnen alleen op basis van medicatie en controle bij medisch specialist worden geïdentificeerd. De verwachting is dat hier een onderschatting is.

Acuut coronair syndroom in kader van hartrevalidatie staat op de lijst van chronische aandoeningen bij de fysiotherapie. Dit betekent dat patiënten met de hartrevalidatie fysiotherapie vergoed krijgen vanuit de Zvw vanaf de 21ste behandeling. Voor vergoeding van de eerste 20 behandelingen zijn deze patiënten afhankelijk van een aanvullende zorgverzekeraar. Ook kan een deel van de patiënten deze kosten zelf betalen. Deze laatste categorie wordt niet teruggevonden in de declaratiegegevens.

Extramurale geneesmiddelen

Acuut coronair syndroom wordt gedefinieerd op basis van de combinatie van de volgende medicatie:

- Trombocytenaggregatieremmer (>180 DDDs)
- Statine (>180 DDDs)
- Betablokker
- RAS-remmer (>180 DDDs)

Deze medicatie wordt ook gebruik voor de preventive van cardiovasculaire aandoeningen in het algemeen. De combinatie van medicatie is specifiek voor acuut coronair syndroom.

Complete patiëntengroep?

Met deze bepaling worden 16,7 per 1000 verzekerden met Acuut Coronair Syndroom geïdentificeerd. 45,5% van deze patiënten is geïdentificeerd o.b.v. medisch specialistische zorg, 65,1% o.b.v. farmaceutische zorg en 1,3% o.b.v. fysiotherapie/oefentherapie/ huidtherapie. De prevalentie van acuut myocard infarct gebaseerd op huisartsenregistratie was 14,6 per 1000 ingeschreven patiënten, inclusief acuut myocard syndroom is dit 38,4. Op basis van ZPD-gegevens wordt 28 per 1000 verzekerden geïdentificeerd. Te verwachten is dat dit met name zit in de onderschatting van stabiele angina pectoris.
